Guide
What Is Calf Reduction Surgery?

Calf reduction surgery improves the size and shape of the calves by shrinking over-developed muscle — most commonly by blocking selected nerves so the bulky muscle naturally slims down. Bulky calf muscles can make legs look short and stout; a smooth calf line completes a longer, more balanced leg line. Korea pioneered this procedure, and Gangnam clinics offer both non-invasive options (Botox, massage therapy — temporary) and permanent solutions (selective neurectomy, partial muscle reduction, calf liposuction for fat-type calves).

Methods
Calf Reduction Methods — Which One Fits You?
| Botox Injection | Selective Neurectomy | Calf Liposuction | |
|---|---|---|---|
| Best for | Mild muscle bulk; first-timers afraid of surgery | Over-developed muscle (permanent fix) | Fat-type calves rather than muscle |
| Result | Temporary (4–6 months) | Permanent | Permanent |
| Downtime | None | 1–2 days rest | A few days; compression stocking |
| Typical cost | From a few hundred USD | $1,800–$2,800 | $1,800–$2,800 |
The right method depends on whether your calves are muscle-dominant or fat-dominant, and how permanent you want the result to be. A photo consultation can tell you which type you have — free and with no pressure.

Recovery
Recovery & Aftercare
- Avoid rubbing the area for 24 hours; a pulling sensation may last about a week.
- Corrective stockings for at least 3 months for the best line.
- Running or hiking from 6–8 weeks after surgery; avoid high heels for 3 months.
- Headache, dizziness, or swelling is rare and usually subsides within 2–3 days.
- No alcohol or smoking for a week — they delay recovery.

Operating-room CCTV: how to request it for your surgery
Under Korea's Medical Service Act Article 38-2 (the Kwon Dae-hee Law, in force since 25 September 2023), operating rooms where patients are put under general anaesthesia or are otherwise unconscious must have CCTV installed. Recording starts only when the patient asks — clinics do not have to offer it first. If your procedure is done under general anaesthesia or sleep (deep) sedation, the law applies to you; procedures done under local anaesthesia alone are not covered.
Ask in writing before surgery day. One line is enough: "I request CCTV recording of my surgery under Article 38-2 of the Medical Service Act" (in Korean: 의료법 제38조의2에 따라 수술 장면 CCTV 촬영을 요청합니다). Footage is kept for at least 30 days and is released only to investigators or courts, or with the consent of you and the medical staff filmed. A surgeon may decline in limited cases such as emergency surgery — if a clinic refuses for a planned surgery, ask why before you book. See our guide to whether plastic surgery in Korea is safe.
